The CDC Foundation is expanding its Teens Linked to Care (TLC) program to enhance youth health and well-being in rural areas of Georgia, Indiana, and Kentucky. Supported by the Elevance Health Foundation, this three-year initiative aims to prevent substance misuse and promote mental wellness among teens aged 13 to 19. Local community partnerships will play a crucial role in establishing advisory boards, hosting health screenings, and launching positive campaigns that cater to the specific needs of their respective regions. The program also connects youth with trusted, youth friendly healthcare services available in their communities.

Participating Sites Include: Georgia: Ben Hill, Jenkins, and Macon Counties. Indiana: Dearborn, Henry, and Scott Counties. Kentucky: Perry County. Scott County, IN, which participated in the original pilot, will serve as a peer mentor site, offering trauma-informed care support and guidance to new partners.
